Hospital transport

CMK Medical Centre will no longer book transport for eligible patients. Only patients who are housebound will be eligible for hospital transport services.

If you are in need of hospital transport and meet the criteria of being housebound, please call 0300 100 0034 to arrange transportation.

For patient’s who are struggling financially please us the low income scheme using the details below:

• NHS prescription charges
• NHS dental treatment charges
• The cost of sight tests, glasses and contact lenses
• The cost of travelling to receive NHS treatment
• NHS wigs and fabric supports (check with your hospital for their arrangements for supplying NHS wigs)

Alternatively you can collect a HC1 form from reception.
