Before filling out a registration form, check you are in our catchment area.
Please note that if you live within the MK9 postcode area, you are within the practice boundary.
You can also find out which GPs you can register with on the NHS website.
Registering with the surgery
To register with the surgery:
- fill out a new patient registration form online
- visit the NHS GP Registration website
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
We will keep your pre-registration form for 30 days, after that you will have to submit another form.
If you are on any repeat medication, you will need to book a medication review with one of the practice pharmacists to reinstate your medication.
When Registration May Be Refused
In line with NHS rules, we may only refuse registration if:
- You live outside our practice boundary, or
- You were previously removed from our list due to violent or aggressive behaviour, or
- There has been a serious and irretrievable breakdown of the doctor–patient relationship
- You were previously removed due to missing too many appointments.
Fair Access to Care
We will never refuse registration because of:
- Age, gender, disability, race, nationality, religion, or sexual orientation
- Medical or mental health conditions
- Language barriers or need for an interpreter
- Homelessness or housing situation
If Your Registration Is Refused
If we are unable to register you:
- We will explain the reason in writing
- We will tell you how to get help registering elsewhere
